Illness/Misadventure Form

At St Stanislaus’ College, we understand that students may sometimes be unable to complete or submit an assessment task on time due to illness or unforeseen misadventure. 

The College Assessment Policy and Procedures booklet outlines the process that must be followed in such circumstances and the conditions under which alternative arrangements may be granted. 

If a student is absent from an assessment or cannot complete a task on time due to illness or misadventure, the appropriate supporting documentation must be provided. This includes the Illness/Misadventure Form.
